Achieve Oxfordshire

Achieve Oxfordshire is the weight management service for Oxfordshire, commissioned by Oxfordshire County Council. They offer a tier 1 weight management advice and information hub. This offer’s individuals and families across the county, evidence-based advice on physical activity, nutrition and healthy lifestyles. You will also be able to use this hub to signpost your patients to find out the latest physical activity groups and activities that are happening in their local area.

The Tier 2 service for all residents of Oxfordshire who meet the eligibility criteria. There are four pathways that a client can choose from: WW, Slimming World, Man v Fat and Lose Weight with Achieve. These programmes run continually throughout the year, in a variety of locations ensuring that it is accessible for all. Our programmes are evidence based and continually updated in accordance with NICE guidelines. Programmes are delivered by qualified and trained practitioners and exercise professionals.

Pathways available to patients:

  • Slimming World – 12 weeks free vouchers
  • Weight Watchers – 12 weeks free vouchers
  • Man v Fat – 14 week football league and weight management – MEN ONLY – £10 refundable deposit
  • Lose Weight with Achieve – 12 week Weight Management Programme including physical activity

Achieve Oxfordshire Entry Criteria:

  • BMI >30 or 27.5 for Asian Populations or individuals with comorbidities
  • BMI>23 for Asian Populations with comorbidities
  • Motivated to make lifestyle changes
  • Oxfordshire Resident

Achieve Oxfordshire Exclusion Criteria:

  • Have accessed this or the previous service in the past 12 months
  • Currently Pregnant
  • Had bariatric surgery in the past 12 months
  • Patients who have been repeatedly referred for weight loss interventions
  • Complex Diabetes
  • Active Substance Misuse
  • Active Bulimia
  • Active Psychosis
  • Untreated Depression or Anxiety
  • Diagnosed Personality Disorder
  • Dementia

Referral Pathway:

  • Patients can be referred into this service through by their GP or other health care professional.
