Social Prescribers/Care Coordinators

 

Social prescribers can help you with many non-medical problems that impact your health and wellbeing. They know a lot about local support services and can help you with:

  • Housing or financial problems
  • Loneliness
  • Practical or emotional support
  • Providing information about local groups, activities or courses

Care Coordinators play an important role within the surgery to proactively identify and work with people, including the frail/elderly and those with long-term conditions, to provide coordination and navigation of care and support across health and care services.
