Job Summary

An exciting opportunity has become available at Hedena Health to join our Integrated Neighborhood (Proactive Care) Team as a Paramedic or Advanced Practitioner.  This team is known as the PACT (Proactive Care Team).

We are seeking an enthusiastic and motivated Paramedic/ANP to join our friendly and supportive team establishing an innovative multi-disciplinary way of working that aims to improve the experience and outcomes of primary health care for the most complex patients at Hedena. The project aims to provide an internal multi-disciplinary team (MDT) that brings together GPs, GP trainees, Practice Nurses, Paramedics, Pharmacists, Social Prescribers, Community Health and Wellbeing Workers and a Mental Health Nurse as needed to integrate proactive care for the most vulnerable or at risk of unplanned hospital admission or rapid deterioration of their health.

The PACT team further integrates with wider external health and social care networks via weekly MDT meetings within our PCN (Manor surgery) which connect with Oxford City Council, social services, housing services, mental health services, district nursing, OUH care of the elderly and ED services as well as urgent hospital at home services. 

This new and innovative approach to patient care, and our ethos of team working and personal/career development, aims to improve health care for the most at need. 

The Paramedic/ANP role is one of direct patient care either through telephone assessments, face to face assessments, and will include a significant element of home visiting. This will not replace acute PCVS home visiting service or district nursing service but will complement this and work alongside these services.

Initial hours available are up to 3 sessions (half days) per week on a fixed term contract until the end of March 2027.

 

Main duties:

The core focus of this role will be working within the PACT based at Bury Knowle Health Centre in Headington. The Proactive Care Team work proactively to improve access to healthcare for the practices highest need patients and their families. The team aim to provide support for patients and their families in the community in their own home environment through both short-term health and social care crisis. This can be during acute illness, chronic condition treatment at home, longer-term frailty, end of life care and to help prevent hospital admission.

The team provides high quality, person centered clinical care for those living with complex health and social needs, addressing individual needs within a patient’s own environment. This is achieved by collaborating with all health and social care agencies and integrating with other services across primary and secondary care. This is a collaborative role which works closely with the Proactive Care Team Coordinator and Clinical GP lead. 

You will need to provide and maintain a high standard of care to patients and support the Practice with other tasks as needed. This will include assessment, diagnosis, treatment and referral of patients depending on their needs, and with the support of daily GP clinical input.

In the course of their work, the Practitioner will:

  • Maintain contemporaneous clinical records for all patient assessments ensuring timely documentation 
  • Deliver evidence-based and patient-focused care, ensuring the patient’s care, dignity, confidentiality and wishes are always respected, working autonomously within their scope of practice and seeking support when needed.
  • Work within the Proactive Care Team to undertake on the day requests from the PACT GP and Care Coordinator/Project Lead to include telephone calls and patient home visits to a protocol mutually agreed with the Clinical Lead GP. 
  • Provide timely, high quality, person centered clinical care to patients. Addressing their needs within current or urgent care needs and long-term self-management.
  • Help to preventing unplanned hospital admissions and Emergency Dept attendances by collaborating with the care coordinator/PACT  to agree the provision of specialist care either by yourself or other specialist care agencies and services across Oxfordshire. 
  • Provide advice and clinical support to the Care Coordinator.
  • Ensure the safety of themselves, colleagues and patients by staying vigilant about infection prevention and control, following practice policies.
 

Personal specifications:

Qualifications and education

Essential

  • Full registration as a Paramedic with the Health and Care Professions Council (HCPC) or as ANP with the Nursing and Midwifery Council (NMC)

Desirable

  • Non medical prescribing qualification
  • Working towards an MSc in Advanced Clinical Practice or equivalent
  • Completion of level 6/7 higher education modules in patient assessment/ management of minor illnesses
  • Completion of additional primary/ urgent care courses such as the College of Paramedics Diploma in Primary and Urgent Care or Royal College of Surgeons Diploma in Urgent Medical Care

Knowledge, skills and experience

Essential

  • Interest and commitment to providing Proactive Care to vulnerable individuals
  • Demonstrable commitment to professional and personal development
  • Experience managing a diverse and constantly changing caseload, prioritising and recognising unwell patients effectively
  • Experience working with Microsoft windows and Microsoft office applications
  • Meets DBS reference standards and has a clear criminal record, in line with the law on spent convictions

Desirable

  • Experience working with a multi-disciplinary team
  • Experience working with Primary Care clinical record systems (e.g EMIS)
  • Experience of working within a primary care setting
